In SCF effort to stop the spread of HIV/AIDS, SCF has partnered with many agencies in the communities that are fighting to stop the spread of HIV/AIDS.
SCF realizes that are other issues affecting those who are HIV positive such as housing, food and childcare. SCF will assist with these issues in order to successfully continue to provide HIV/AIDS services. Most importantly, SCF will protect confidentiality.
SCF’s goals are to provide services that are best suited for your needs and want you to feel comfortable visiting our client-centered facility.
Case Management Services
The Street Corner Foundation’s case management will encompass a range of client-centered services that will link clients with health care, psychosocial and other services to ensure timely, coordinated access to medically appropriate levels of healthcare, health and support services, continuity of care, ongoing assessment of the client's and other family members' needs and personal support systems.
Identified HIV positive persons will receive help through referral services, such as housing, support groups, medical care and others. Key activities include an initial comprehensive assessment of the client's needs and personal support systems; development of a comprehensive, individualized service plan; coordination of services required to implement the plan; client monitoring to assess the efficacy of the plan; and periodic re-evaluation and revision of the plan as necessary over the life of the client. Services may include client-specific advocacy and/or review of utilization of services.
Information/ Education & Referrals
HIV/AIDS education, information, public policies, AIDS leadership and referrals to other HIV/AIDS programs.
Comprehensive Services
HIV Testing
On-site confidential HIV Testing will be available. SCF will not release your status or medical information without your written permission.
Hotlines/Crisis Help
Staff will be available 24 hours 7 days a week to support persons who call seeking HIV/AIDS counseling or advice or support.
Support Groups
Host support groups for individuals who are infected or effected by HIV/AIDS.
Health Promotions
Activities in the community that will provide education and awareness on HIV/AIDS such as the Big Tigger’s Annual Celebrity Classic.
HIV/Awareness Before & After School Program
Educational services on HIV/AIDS for middle and high students.
HIV/AIDS Awareness College Tour
Educational services on HIV/AIDS Awareness for college students.
Community Street Corner Foundation Team
HIV/AIDS Awareness information disseminated through flyers, newsletters, internet and media to the community on upcoming events for The Street Corner Health Promotions.
Housing
SCF will provide housing referrals for HIV positive individuals.
Condom Distribution Initiative Campaign
SCF will distribute Street Corner Foundation Condoms throughout the communities in its efforts to help prevent the spread of HIV/AIDS.
Street Corner Foundation Community Forums
SCF will host forums within the communities to discuss ways in which SCF and the communities can continue their efforts in fighting the spread of HIV/AIDS. |
